Enter your Member Number (e.g., 1234).

For spouses, append an 1 (e.g., 1234-1) to the member number. For dependents (juniors), append a 2, 3, etc. (1234-2).

*If your member number has less than four digits , please populate with 0 e.g., 0001

Enter your temporary password, which is your last name in lower case.

Username: 1234
Password: smith

Once you log in, please update your password by following the on-screen instructions.

Forgot Password
